We drove back from Tahoe yesterday so that I could make my appointment with Dr. F this morning over at Stanford. After my last visit in late September, we'd decided that we would see how the brace helped, along with lots of swimming on my part, before we closed on any other course of action. Well, the brace has definitely had an impact - when I wear it, I don't feel the catching and clicking and as much of the pain, though I still have some pretty significant swelling at times. So I was interested in what Dr. F was going to say.

As I was walking into the building, he was driving into the parking lot, so he got a chance to see me walk for more than the distance between the waiting room and the exam room, which in that office is a gauntlet anyway, so it's not like I walk in much of a straight line. I talked to his assistant first, who noticed that I was still limping and that it was swollen, and I let him know that the brace did seem to be helping. Dr. F came in a few minutes later and said, "OK, what are we going to do with this thing?" He apparently didn't take seriously my suggestion of "Shoot it." I can't imagine why. He said he thought he'd figured out what he wanted us to do, but then he saw me walking in the parking lot and realized that we needed to talk about "all" of the options available. We discussed 4 or 5 different paths, including synthetic protein injections (which I gave him an over-the-top-of-the-glasses-are-you-really-serious look for, resulting in him laughing and saying that he gets that enough at home and doesn't need it from his patients, too), continued PT, doing nothing, a combination of all of the above, and another surgery. He did what all good doctors do and poked/pulled/pushed what hurts, and then went to check something on the MRI again as well as talk to one of the other doctors in the office. When he came back, he showed me how different both knee were working - with the right (good) knee, he could move the kneecap around basically with a finger and not much effort; with the left (very very bad) knee, he had to use the heel of his hand to push and the tips of his fingers to pull the kneecap in any direction at all. There's apparently a ton of pressure being put on the outside half of the kneecap by the muscles and the way the kneecap tracks. Ultimately, all of the other options we discussed would either just mask the issue or would lead to another surgery at some undetermined point. Since I've already had a year screwed over by this stupid knee, I would prefer to just get it over with and not spend time wondering when it was going to freak out again and send me back to Dr. F. So he's going go in through the scope, and, at a "minimum" cut most of the muscles that attach to the outside of my kneecap (along with any scar tissue that's in the same area) and "release" the kneecap from that pressure. There are other things he might have to do once he sees how it moves after that is complete during the surgery, but we're hoping this is it. He also thinks that we will proactively do the synthetic protein injections once it has healed a bit after surgery to give it a bit of a jumpstart, and he will have me continue using the brace as it heals and after that even, to ensure that as scar tissue forms, it happens in the correct place.

He said that as long as we're doing it, we should do it before the end of the year (to take advantage of my already-long ago met insurance deductible), and just to get it over with. He took me out to his scheduler who said, "You again?" with a little laugh, and then he disappeared to see another patient (that was after about 25-30 minutes with him - again, quite lengthy for this office). When she opened his schedule, she said, "the first time he has available is Jan 14." I told her that he said he would do it before the end of the year, and she told me to hang on a second while she went to check with him. When she came back, she said that he was going to come in on a Monday for this one (he usually only does surgery on Wednesday), so we could do it on Dec 15 or 22. Since my company is shut down again for the week of 12/22 and 12/29, I figured I might as well do it that first day and be a vegetable for those two weeks. On the bright side, I'm going to really be seeing Santa and flying reindeer thanks to the painkillers, and I'm out of cooking, table-clearing, and dish duty on Christmas....

And that, ladies and gents, is how I happened upon my fourth knee surgery on my oh-so-favorite left knee. Oh, and my dad called this afternoon and said that he was on the way home from his ortho doc with a fancy-schmancy boot on his foot because he found out he has a broken bone in there after walking around in pain for a month and a half.
